If you're a new patient, we ask that you complete any forms that require your personal or medical information before your first visit.

When you come for your appointment, please bring the following items
 
  • A picture ID (e.g., driver’s license, military ID, school ID, etc.)
  • Your insurance card(s)
  • The actual bottles of medications and supplements you are currently taking, or a medication list
  • Any medical records from previous physicians and hospitals​​
