What insurance do you accept?
We accept Medicare and various other insurance plans. Any deductibles or co-payments for medical services are processed in the same manner as those at a physician’s office.
We currently participate in the following insurance plans

- Florida Blue Cross Blue Shield
- Freedom Health
- Humana
- Medicare
- Optimum
- Ultimate
- United Healthcare
- Any PPO Insurance
If your insurance plan is not listed above, you will be responsible for payment in full. As a courtesy, we will file your initial insurance claim, but if payment is not received within 30 days, you will be responsible for the balance.
Do you accept Self-Pay??
Self-pay accounts refer to patients without insurance coverage, patients whose insurance plans do not include our practice or those who do not have an insurance card on file with us. Liability and workers' compensation cases will also be considered self-pay accounts.
